Our services are not intended for the diagnosis or treatment of any medical, physical or psychological condition. Hypnotherapy, and any other services provided are for educational, empowerment and self-improvement purposes only. If you have an ongoing physical illness, mental disability or mental illness, please consult a medical doctor, psychiatrist or psychologist or other health professional as necessary, you are in control of your overall health at all times.
Fees: $160 first/initial session unless package or other arrangement is purchased prior or on the day of. This includes the comprehensive assessment done prior to your appointment by your therapist upon return of your Confidential History Forms. Payment is due in full at the time of the session.
Prepaid regular sessions can be purchased in quantities of 5 at $680 (savings of $120) Full payment must be made at the time of the first session to receive the discount. Personalized recordings are included free of charge where beneficial to the client as deemed by the therapist.
Payments can be made via cash, bank transfer, Stripe, Square, Pay ID and Afterpay. Cash is always appreciated.
Cancellation Policy: Your appointment time is reserved exclusively for you. Please arrive promptly to obtain your full session but not excessively early as this may impede on another’s session. If you must cancel or reschedule, we require a minimum 24 hours’ notice. Unless cancelled prior to 24 hours, you are financially responsible for $50 of the scheduled fee. If you must cancel or reschedule due to an emergency, please notify us as soon as possible. Any cancellations prior to 24 hours will be refunded in full minus the fees from the chosen method of payment.
Confidentiality: All hypnosis sessions and notes are completely confidential. Vivify Utopia, Raw Hypnotherapy and Hypnotherapy Whitsundays will take minimal notes and only those related directly to the therapy in order to provide a better service to you, the customer, no note taking at all can be requested by the client. This is your session and your choice. We pride ourselves on total confidentiality and complete privacy. Your therapist will be with you 100% during your session times and our client history and appointment times and notes are stored safely and in accordance with regulations.

We will not release any information without written authorization from you, except as required by law or duty of care in regards to safety towards yourself or others.
Client Consent and Release: I am of legal age and in consideration of my acceptance as a participant in hypnosis and hypnotherapy sessions, yoga, any training, seminars, groups, circles, retreats or any other event hosted by Vivify Utopia, Raw Hypnotherapy, Hypnotherapy Whitsundays or Maia Hogg, I for myself, my heirs, executors, administrators and assignees, do hereby release and discharge Maia Hogg, Vivify Utopia, Raw Hypnotherapy, Hypnotherapy Whitsundays and/or any of their employees, family or all other participants from all claims of damages, copyright, demands or actions whatsoever in any manner arising from my participation. Further, I understand that any audio recordings which are made under all circumstances, cannot be reproduced, shared or uploaded in any manner and are under copyright of Maia Hogg and Maia Hogg retains the copyright of these recordings indefinitely.
We facilitate to the absolute best of our ability, the best possible experiences, results and growth for our clients, we have extensive training in many areas including full first aid and take our duty of care of the utmost importance though, this is your session, you are in complete control and responsible for yourself and your actions, during and as a result of any sessions at all times.
I declare that as a client, I have read this consent and release and that I fully understand and agree to all the terms described above.
